‘Calamity of errors’ led to state’s early prisoner releases

Flawed advice, ignored emails, and a lack of communication allowed the state to continue mistakenly releasing prisoners early for years after the problem was discovered in 2012, according to a report from Gov. Jay Inslee released Thursday.
