Mechanical, human error led to second jail escape in 2 years

A mix of mechanical and human error led to the second inmate escape in two years from the same Sandusky County jail doorway, county officials said Thursday. Nicholas Maldonado, 32, remained on the lam about an hour after he escaped through the same door during recreational time in March, 2016.
