Bridging hospital to home: How a North York health hub is developing transitional care

A partnership between CAMH and LOFT Community Services is transforming approximately 20,000-square-feet of Champagne Centre into a permanent transitional-care environment, with 40 beds for patients who no longer require acute hospital care but are not yet ready to return home.
