Quebec teen's hospital death from morphine 'highly preventable,' coroner finds

MONTREAL — When Jimmy Lee Durocher, a 17-year-old in otherwise perfect health, went into the operating room Jan. 14 at a hospital in Joliette, Que., his family expected it would be a routine appendix removal.
