The “most probable” explanation for the death in 2016 of 23-month-old cancer patient being treated for cancer at Ste-Justine Hospital was that a nurse accidentally administered an injection of potassium rather than saline to the child after he had received a previous injection of medication, a Quebec coroner has concluded.
