Quebec's youth-protection service failed child who died: Commission

Quebec’s Human Rights and Youth Rights Commission found serious shortcomings in the process of protecting a Saguenay—Lac-St-Jean child who died after his case was reported to the Direction de la protection de la jeunesse (DPJ), the province’s youth-protection service.
